What documents are required to hire an employee?

A package of documents for a candidate for an employment contract («Umowa o pracę»):

Candidate's personal questionnaire;
Employment contract for a trial period / for a definite period / for an indefinite period;
Medical report;
Consent to the processing of personal data;
Statement on exercising parental rights;
Application for reporting family members to health insurance.

What documents are required for employment under civil law contracts («Umowa zlecenia» / «Umowa o dzieło»)?

HR documents look a bit different in the case of people employed under a civil law contract. The employer is not obliged to keep personal files, but is obliged to provide the following documents:

Candidate's personal questionnaire;
Agency contract/contract for specific work/orders;
Referral to training in the field of labor protection (if required by the type of work).

In the case of employing an employee residing in Poland, a Polish address of residence and a PESEL identification number are required.
